DOT Physical Exams are $90. 

Payment can be: cash, debit or credit card,  Venmo.

Your exam includes: A copy of the medical exam form and a laminated DOT medical card. 

Dr. June will register, for you, your DOT Physical with the required National Registry. 

Federal DOT Urine/Hair Drug Testing: $80
